📑 Purpose

This policy explains how patient personal data is managed within the practice, the IT systems we use, and the rights patients have regarding their information.

🖥️ What We Use

  • 🔗 Referrals – sent using your NHS number to make sure they are accurate.

  • 📅 Appointments – you can book or cancel routine appointments online.

  • 💊 Repeat Prescriptions – patients can request repeat prescriptions securely using the NHS App.

  • 📂 Summary Care Record (SCR) – your current medication and allergies are uploaded to the NHS “spine” so doctors elsewhere can see them if needed.

  • 🔄 GP to GP Transfers – your records move securely with you if you register at another practice.

  • 👀 Patient Access – you can view your medical records online.

⚖️ Patient Rights and Choices

  • Patients may request access to their medical records.

  • Patients may choose to opt out of the local or national Summary Care Record if they wish.

  • Patients can also request that online access to their records be enabled.

  • To exercise these rights, please contact the practice for assistance.

🔐 Keeping Your Data Safe

  • All personal data is held securely and protected against unauthorised access.

  • Staff are trained in confidentiality and data protection.

  • We comply with the Data Protection Act (2018) and UK GDPR.

📝 Patient Registration for Online Access

  • Patients who are not already registered for online access should visit our online services page for more information.

  • Once registered, you can book appointments, order prescriptions via the NHS App, and view your records securely.

⚖️ Your Choices

  • You can ask us to enable online access to your medical records.

  • You can choose to opt out of the local or national Summary Care Record if you prefer.
