Register as a New Patient

We’re always happy to welcome new patients who live within our practice area. Most postcodes beginning with S2 and S12 are included, but please check Find a GP - NHS to make sure we're the right practice for you.

Your medical care starts from the day you register. However, we need your details to transfer before we can issue medication, so please make sure you have at least one month’s supply from your previous practice to avoid any gaps.

New Patient Registration form

Registering with us is simple. You can visit reception for a paper form or complete the online form. We’ll also ask you to fill in a short medical questionnaire so we can provide the best care from day one.

Having your NHS number makes registration easier. You can find it here: Find your NHS number - NHS

Important:

  • We cannot register a child unless a parent or guardian is also registered with us.
  • You do not need proof of identity, immigration status, or address to register. We will never refuse registration because you cannot provide these documents.

Stay up to date with practice news and updates on our Facebook page: https://www.facebook.com/eastbankmedicalcentre/

Temporary Patient Registrations

If you become unwell while away from home or are not registered with a GP but need care, you can receive emergency treatment from a local GP practice for up to 14 days. After this, you’ll need to register as a temporary or permanent patient.

You can stay registered as a temporary patient for up to three months. This means you’ll be added to the local practice list but remain a patient of your usual GP. After three months, you’ll need to re-register as temporary or consider registering permanently.

To register as a temporary patient, simply contact the local practice you wish to use. You can find practices here: Find a GP - NHS

Please note:

  • Practices are not required to accept temporary patients, but they must offer emergency treatment if needed.
  • You cannot register as a temporary patient at a practice in the same town or area where you are already registered.

Information for Non-English Speakers

Moving to a new country can feel overwhelming, especially when it comes to healthcare. These fact sheets are here to help you understand how the NHS works and how to access the care you need.

They explain:

  • The role of GPs and why they are your first point of contact for most health services.
  • How to register with a GP practice.
  • What to do in an emergency.

The information is written in clear, simple language and has been carefully tested with real users to make sure it’s easy to understand.

You can view the leaflets in your preferred language below.
