Summary Care Record Policy

Purpose

Your patient record is held securely and confidentially on the electronic system at your GP practice. If you require treatment in another NHS healthcare setting—such as an Emergency Department or Minor Injury Unit—those treating you can provide better care if some of your GP information is available to them.

What is the Summary Care Record?

The Summary Care Record (SCR) is a secure, electronic record used nationally across England to share essential health information with authorised healthcare professionals involved in your care.

Access and Consent

  • Information will only be accessed by authorised healthcare professionals directly involved in your treatment.
  • Your permission will be requested before access, unless:
    • You are unable to give consent, and
    • There is a clinical reason for access.

Opting Out

  • If you would like to opt out of having a Summary Care Record, please ask reception for an opt-out form.
  • Parents or guardians can request to opt out for children under 16. However, the GP has the final decision based on their duty of care.
  • If you believe your child can understand, please share this information with them.

Who Has Access?

Across all healthcare settings in England, including:

  • Urgent care
  • Community care
  • Outpatient departments

Please note: Access is only granted if you are under the care of the clinician requesting the information.

Content of the Summary Care Record

  • Current medications
  • Allergies
  • Any adverse reactions to medicines
  • Additional information (upon request to your GP)
