• 📄 Support letters, forms, or medical reports for:

  • 💼 Employment

  • 🚗 Driving licences (DVLA)

  • 🛡️ Insurance purposes

  • ✈️ Travel or holiday cancellations

  • 🏅 Sports or leisure activities

• The GP will inform patients of the fee before proceeding.

• Requests will be processed within 30 working days.

We do not carry out medical examinations for:

  • A signed patient consent form is required for all requests, unless the request comes directly from the patient.

• 📬 Submitting Requests

  • Requests must be made in writing, preferably via:

    • AccuRx

    • Email: syicb-sheffield.eastbankmc@nhs.net

    • Letter

  • Requests must include:

    • A clear reason for the request

    • Patient details

    • Preferred method of receiving the report (email or paper copy for collection)

• 📧 Delivery of Reports

  • Patients must provide an email address they are happy for the report to be sent to, or specify if they require a printed paper copy for collection.

  • Please be aware that reception staff are not involved in processing medical report requests.

  • All requests are managed by our Reports Team, who will ensure your request is handled correctly.

  • To avoid delays, please submit requests directly in writing (AccuRx, email, or letter).
